The essentials
Your monthly membership gives you a direct relationship with your physician for comprehensive primary care.
Membership is not health insurance and does not take the place of coverage for hospital, emergency, or specialty care.
No annual contracts, co-pays, deductible charges, or surprise bills for your membership.
Care that meets you where you are
Membership is built around time with your physician, a simpler way to reach your care team, and room to make decisions together.

Included with membership
Home visits are available within a five-mile radius of the clinic, when deemed appropriate and necessary at the physician’s discretion.
