What your membership does not include.
Your membership covers primary care from Sierra Prieta Health. It is not health insurance and does not cover:
- Emergency department or hospital care
- Specialist care or services provided by other medical practices
- The direct cost of labs, prescriptions, imaging, or supplements
Members may receive negotiated discounts on many labs, medications, and supplements. Sierra Prieta Health is not a 24/7 practice or an emergency service. For a medical emergency, call 911 or go to the nearest emergency department.

Before you join.
Do I need insurance to become a patient?
No. Insurance is not required to become a Sierra Prieta Health patient, and the practice does not bill or accept insurance for membership fees.
Is the membership health insurance?
No. Direct Primary Care membership is not health insurance. We recommend that patients consider coverage for significant known or unforeseen medical needs outside primary care.
Is Sierra Prieta Health available 24/7?
No. Sierra Prieta Health is not a 24/7 practice and is not an emergency service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Can I use HSA or FSA funds?
HSA accounts can be used for monthly membership fees effective January 1, 2026. HSA and FSA funds can typically be used for labs, medications, and imaging that we order or prescribe. Please consult your employer or tax adviser about your eligibility.
What about Medicare, Medicare Advantage, or AHCCCS?
We can order, prescribe, and refer for covered Medicare services that are not included in membership fees. For Medicare Advantage HMO, please contact your insurance company to confirm it will accept Dr. Maiolo’s orders. At this time, we cannot order, prescribe, or refer through AHCCCS.
