You have the right to get a copy of your paper or electronic medical record, ask us to correct your record, request confidential communications, ask us to limit certain information we share, get a list of certain disclosures, receive a copy of this notice, choose someone to act for you when legally authorized, and file a complaint if you believe your privacy rights have been violated.

Our uses and disclosures

We may use and share your health information to treat you, run our practice, bill for services when applicable, and comply with law. We may also share information for public health and safety activities, to respond to organ and tissue donation requests, to work with a medical examiner or funeral director, for workers’ compensation, law enforcement, government requests, or lawsuits and legal actions when permitted or required by law.

We may share relevant information with family, friends, or others involved in your care when you agree or when you do not object. We will obtain your written permission before using or sharing your information for purposes not otherwise described in this notice, except where law allows or requires us to do so.
